Selection of the End-Expiratory Pressure for Mechanical Respiratory Support (Review)

End-expiratory pressure remains one of the few parameters mechanical respiratory support whose values have not been strictly regulated using evidence-based approach. The absence «gold standard» for end-expiratory optimization together with its obvious significant contribution to efficiency and safety has driven search optimal method choosing several decades. Aim review : identify methods determining based on analysis positive negative effects in used strategies support. Material . We analyzed 165 papers from PubMed, Scopus, RSCI databases medical biological publications. Among them we selected 86 sources that most completely covered following subjects: support, pressure, recruitment, ventilation-perfusion relationships, metabolography, gas analysis. Results. outlined main regard both lung biomechanical characteristics pulmonary perfusion. evolution views was reviewed emphasis a certain «fixation» scientific community recent decades concerning opening alveoli. promising techniques diffusion capacity lungs were presented. Conclusion. Focusing prevents advancing pressure. Dynamic assessment provides new perspective issue, offering additional ways development standard».
